How to Submit a Request for Service:
If you are interested in any of our services, you can submit
a Request for Service by
using either of the following options:
1.) Instructions on how to fax or mail a
hardcopy of the Service Request Form:

Click on the

Request for Service Form.

The Request for
Service Form will open in Microsoft Word.

Select File -
Print.

Once the form is
printed, fill it out as completely as possible and fax to 610-454-9324.

2.) Instructions on how to e-mail a
softcopy of the Service Request Form:

Click on the

Request for Service Form.

The Request for
Service Form will open in Microsoft Word.

Download the file
onto your computer by selecting File - Save As.

Open the file in Microsoft Word.
-
Hit the INS key on your keyboard. This
will allow you to overwrite the lines
on the form with the appropriate information.
